Admission Process
Prior to admission, all individuals need to complete the following:
- Completed Application including:
- Completed Referral Form and Application Form
- Completed Medical Report
- Upon completion of the application, GMCS staff will complete an internal assessment including:
- Assessment of medical appropriateness
- Identify transportation and nutrition needs
- Individual goals and initial plan
- Set start date and identify funding source
